How to request Harney District Hospital medical or billing records for someone else (third party)

Please complete the authorization form above to request information. Be sure to include documentation that legally supports your right to request records on behalf of the patient. Examples of this documentation include Healthcare Power of Attorney that refers to medical records, Legal Guardianship, Court Order, or a designation of Healthcare Representative in an Advance Directive.

Please note: Many Durable Power of Attorney documents do not include language that allows a designated person to request medical records on a patient’s behalf.

MyChart

The Harney District Hospital MyChart patient portal offers you personalized and secure online access to portions of your medical records, as well as multiple tools to help you manage your care.  MyChart is accessible through a web browser or mobile application. You may access MyChart here.

With MyChart, you can:

  • View test results
    Once available, see results compared to standard ranges and view trends over time
  • Communicate with your doctor
    Send and receive messages with your care team about nonurgent issues
  • Schedule appointments with Family Care clinic
    Manage upcoming appointments and view details of past visits, including comments and instructions from your doctor
  • Pay bills online
    View billing statements and pay bills and co-pays with a credit card

You can also:

  • Contact customer service
  • Request a referral to a specialist
  • Review details of care advice calls
  • Request a renewal of current medication
  • See details about future appointments, including instructions and driving directions
  • Confirm, cancel, and reschedule appointments
  • Search open appointment availability
  • Move appointments to an earlier date if an opening becomes available
  • Complete most of the check-in process from home — like paying co-pays, verifying allergies and medications, and completing questionnaires

You may request additional medical records through your MyChart account

Please log into your MyChart account and select the “Request Records” button. Complete the online form to request your records. Only records for services that occurred on or after April 7, 2018 may be sent to you through MyChart.

Note: Billing or radiology images will not be available through MyChart, but you may request these items by completing an Authorization to Use and Disclose Protected Health Information.

Your records will be released to your MyChart account. Please download or print them for your future use, as they will expire 30 days after they have been released to you.

Proxy access to MyChart

If you have access to your family members’ medical records, you can view most of the information in their records in the same way that you view your own.
Some things that might be particularly useful include:

  • Viewing or printing your child’s immunization record
  • Viewing your child’s growth charts
  • Viewing a family member’s test results

Proxy access for a minor ages 0 – 13: If you are a parent or legal guardian with a Harney District Hospital MyChart account, log in to your MyChart account and electronically request proxy access for your child’s MyChart account from the MyChart Homepage. If you do not have a Harney District Hospital account, please fill out the Minor Proxy Access Authorization Form and bring it to your child’s next visit or mail to the address noted on the form. Step-by-step instructions can be found here. You will be asked to log in before viewing the document.

Proxy access for a minor ages 14 – 17: The minor may send friends or family an invitation (from their MyChart account) to gain proxy access. From the minor’s MyChart account, minor can access Health > Share My Record > Friends and family access to invite another person to view their MyChart record. If the minor hasn’t created a MyChart account nor wishes to, complete a copy of the Minor Proxy Access Authorization Form and bring it to your child’s next visit or mail to the address noted on the form. You will be asked to log in before viewing the document.

Proxy access for adults ages 18 and older: The patient may send friends or family an invitation (from their MyChart account) to gain proxy access. From their MyChart account, patient can access Health > Share My Record > Friends and family access to invite another person to view their MyChart record. You will be asked to log in before viewing the document.

If the patient hasn’t created a MyChart account nor wishes to, complete a copy of the Adult Proxy Access Authorization Form and mail to the address noted on the form.

How to allow a third-party app to access your health record 

Patients of providers who use Epic software are able to connect third-party applications (apps) to retrieve parts of their health record for their own personal use. Examples of data that can be pulled into an app include lab results, allergies, medications, and immunization history.

In order to authenticate an app to retrieve your health data, follow the steps outlined on Epic’s website. (Note that images may not reflect Harney District Hospital’s exact layout or content for each step in the process).

Electronic Health Information (EHI) export

You may request an electronic document that includes all information within your entire medical record. This large document acts as an interface between two electronic health systems. Each time an EHI export is requested, the system will capture your most recent visit as well as everything else in your medical record. Although it is computer friendly, this document takes up a lot of space. We advise that you request this document in electronic PDF format, as it is the most user friendly.

How to request a birth or death certificate

Even if the patient was born or has passed away at Harney District Hospital, birth and death certificates are maintained by the state of Oregon. To request a copy of a birth or death certificate, please visit the Oregon Vital Statistics website.

Frequently Asked Questions (FAQs)

How much does it cost to get my records?

The fee depends on the format of the records and the number of pages released. Harney District Hospital’s Release of Information department can provide you with copies of your records in paper or electronic format. There is an additional cost for radiology images on a CD.  (Standard copy fees will apply subject to federal and state regulations).

How long will it take to get my records?

Though federal and state laws allow up to 30 days for processing, our goal is to have your medical records request completed as soon as possible. Once we have completed your request, our team will either mail you an invoice (if there is a fee) or call you to alert you that the records are ready.

If you are asking that your records are sent to a health care provider, it is often faster to ask the provider’s office to request that information be sent directly to them.

How will I receive my records?

If there is a charge for records, you will be mailed a billing invoice. You may either pay over the phone or by mail. We accept credit/debit, cash and checks.

Once your invoice is paid, we will mail your records to you or you can call 541-573-8339 or 541-573-6054 to discuss other methods.

How do I request an amendment to my record?

If you believe there is an error in your medical records, you may request that Harney District Hospital review your record. If approved, an amendment may be made. A request for amendment is not a guarantee of the change. However, you will be contacted regarding the result of the review process.
